Thank you for your interest in becoming an agent with Advocate Health a Managing General Agent (MGA) for Humana Inc.

Right now is one of the most exciting times ever when it comes to senior health insurance plans.

Moving forward is as easy as one, two, three: 1) Humana Appointment, 2) Humana Certification, and 3) Advocate On-boarding Process which will include your commission agreement.

1) APPOINTMENT WITH HUMANA:

The first step toward appointment with Humana starts by completing the Humana Contract and must take place before anything else

Humana Contract

This form is required by Humana and may be filled out by clicking here.

After completion of the form, certain pages will need to be signed and faxed to Advocate Health LLC, as per the instructions specified on the first page of the contract.

2) HUMANA CERTIFICATION PROCESS:

Once appointed but before you can sell you must be trained and certified by Humana to represent their specific products

Pre-Work for Certification Class

Prior to attending the Certification Class certain you must complete the online Pre-Work and bring your certificate of completion to the Certification Class. This information is available online here.

Certification Class

Certification School is a full day course to teach you the specifics of Humana Products. You may locate the current Schedules and Enroll in the Certification Class by contacting your agent program manager. Click here for a by state directory

3) ADVOCATE ON-BOARDING PROCESS:

Advocate Contract

This contract is required by Advocate Health LLC and may be filled out by clicking here.

After completion of the contract, it must be faxed to Advocate Health LLC.

Advocate Commission Agreement

This agreement will also be provided to you after approval of the Humana contracted by your delegated Advocate representative.

IRS Form W-9

IRS Form W-9 is required to be completed by all entities/individuals to whom we will be making payments. This form may be filled out online by clicking here.
